New Patients
To schedule an appointment at Barnard Family Health Centers in Cypress, Texas, please call us at (281) 469-7704 or request an appointment online using our interactive appointment request form below. Our appointment scheduler will respond to your request within 48 hours.
Be sure to bring any diagnostic films or other test results that may have been ordered or performed by another physician.
Once you have made your initial appointment, you may pre-register by logging in to the patient portal with log-in credentials that were sent to you via e-mail to complete the pre-registration reprocess. Pre-registration must be completed prior to your appointment. Please note that you must bring a current insurance card to all appointments as pictures will not be accepted.
